Patients can self-refer to the community nursing service via Local Access Point (LAP), only if the referral has not been generated from a GP surgery.

- Cannock Local Access Point is available 08:00 – 18:00 hours Monday to Friday and 08:00-16:00 on Saturday
The referral will be triaged by LAP and CIS and maybe signposted to the most appropriate service. Triage may contact the GP practice and ask for further information including a patient summary.
GP Surgeries can also refer patients to the community nursing service via the LAP and will require the following information:
- Patient details
- Clinical history and reason for referral
- Patient summary will be requested.
All patients must be either housebound or shielding due to COVID 19. Shielding patients supported by the community nursing team are unable to access the GP practice either under their own esteem or with assistance from a member of their support bubble. Patients with wounds to the lower limb that have been treated for 4-6 weeks can be referred but will need to be informed if they are non-housebound they will be offered an appointment to attend one of our ambulatory clinics for assessment and formulation of management plan.
